Documenting our activities and sharing our progress with key stakeholders is a major priority of the HIV ACCESS PCMH Demonstration Project. By communicating via multiple methods (e-newsletters, short videos, webinars, in person presentations and meetings, etc.), we aim to build momentum toward PCMH transformation both within our project and in our wider community.
Two of our primary means communicating our project activities are our monthly Home Improvement Bulletins and our Voices from the Field video series.
The Home Improvement Bulletin is a monthly e-mail newsletter shared with both local stakeholders (e.g., staff at participating clinic sites) and the broader PCMH community (e.g., other health centers embarking on PCMH transformation). In each Bulletin we highlight major project activities and successes, share resources, publicize relevant events and training opportunities, and introduce project staff to our community. In our Voices from the Field video series, we capture observations on the Patient Centered Medical Home from multiple perspectives, from executive leadership to front line staff. These short videos highlight various aspects of PCMH transformation and panel management, told by the staff making it happen!
