Tiburcio Vasquez Health Center Expands Their Care Transformation Model

Earlier this month, Tiburcio Vasquez Health Center (TVHC) celebrated the expansion of their Care Transformation model in their Pediatrics department at their Mission Health Center.
The Care Transformation pilot program, now operating with two Care Teams at Hayward Pediatrics, aims to improve both patient access and staff satisfaction by implementing a Care Team model for each patient. This model combines the strengths of TVHC and insights from successful healthcare systems to foster a gratifying work environment for staff while building stronger relationships between care teams and patients
Care Teams consist of a dedicated Primary Care Provider (PCP) working in partnership with a Medical Assistant, Resources Specialist and Panel Specialist.
Additionally, this new launch in Hayward Pediatrics will be piloting an LVN Panel Specialist to provide unique support to their patients and team members alike.
To kick off the launch, Chief Operations Officer Kathryn Horner hosted a lunch for staff members involved in the ongoing pilot program to celebrate the successful expansion.
“Care Transformation has been such an amazing way of caring for our patients because they now have a great team that is providing support in many different areas,” said Pediatric Nurse Practitioner Kally Rogers at the launch event. “As a provider, I also feel very supported by Care Transformation as I am able to focus on my role and know that I have my team to help with any additional resources and assistance that families may need.”
