Axis Bridge Offers Crucial Support as School Year Begins
Contributed by Axis Community Health
As the new school year begins, and the stress of the return to school settles in, many parents are often on the lookout for services for their kids; and additionally, may need support for themselves. Axis Community Health is proud to remind the Tri-Valley community of the Axis Bridge mental health urgent care program, a free resource available for children and adults, ages 5 and up. The Axis Bridge team is available in-person and virtually. For more information on hours of operation, please call (925) 568-2080 or Axis Bridge.
Axis Community Health’s Axis Bridge: An Effective Urgent Crisis Intervention
For over 52 years, Axis Community Health has been a steadfast presence in the Tri-Valley community, delivering essential healthcare services to those who need them most.
In July 2021, we launched the innovative Axis Bridge program, a behavioral health urgent care initiative that has since become a cornerstone of our services. Axis Bridge was formed out of a need identified by our community leaders and our relevant experience, which demonstrated a high demand for urgent mental health intervention in this area for all, regardless of insurance type. The program primarily receives patient referrals from Tri-Valley school districts, police departments, and Stanford Health Care Tri-Valley Hospital, further strengthening our ties with our community and local institutions. The success of the Axis Bridge program is evident in its tangible impact on hospitalization rate – referring less than 1% of patients presenting to our program for involuntary hospitalizations (aka 5150). Our program has proven successful at providing same-day/next-day support to those in need, while also assisting with the often daunting task of securing long-term treatment.
Axis Bridge’s Structure
Our program provides mental health services to individuals aged 5 and up in our community. Services include assistance from care managers around social needs and referrals, assistance navigating the insurance system and locating long-term care, counseling and education by licensed mental health providers and psychiatry, aimed at handling hospital refills or providing medication to stabilize symptoms. Community members accessing our services are struggling in a variety of ways, some of which include depression, anxiety/panic attacks, recent traumatic experience, mood disorders or general difficulties carrying on daily tasks and responsibilities. If you are having difficulty knowing where to start, give us a call and we can help you identify next steps and options.
This program meets a care gap within our community and can provide services to all members regardless of insurance type.
Axis Bridge services are currently being offered in-person and via virtual appointments, depending upon the need of the community member.
