DESCRIPTION:In conjunction with the UCSF Center for Excellence in Primary Care (CEPC)\, CHCN will again be offering the PHASE Health Coach Training to clinic staff on August 1st and 2nd\, 2017.  We received lots of positive feedback from the last training in February\, and we are excited to offer the training with two new facilitators who will be trained from within our clinics. \nThere were some staff who were interested in the training and were unable to make it in February.  We also know new staff have been hired at the clinics\, who might benefit from this training.  We’d like to first offer the training to those who have not been able to attend\, but may open it up to those who would like a refresher\, if we are able to accommodate them.

Participants will gain a healthy respect for the long term consequences of HBV as well as the risk factors for reactivation. We will discuss who should be referred for treatment and how to effectively screen for hepatocellular carcinoma in this patient population. \nLearning Objectives:\n1. DESCRIPTION:We have been working with Drs. Jan Diamond and Sophy Wong on efforts to increase access to hepatitis C treatment at our community health centers since 2015. We have heard from several clinics that patient navigators\, case managers\, MAs\, and community health workers would like training on hepatitis C basics\, as well as skills-building on patient navigation for hep C. \nParticipants: Hep C patient navigators\, case managers\, MAs supporting providers with hep C\, community health workers\, and other front line staff supporting hep C testing and treatment \nOur tentative agenda for this first meeting includes: \n* Hepatitis C 101 for patient navigators and other support staff \n* Patient navigation basics for hepatitis C\, including patient tracking tools \n* Discussion of sample cases \n* Q&A on hep C \nWe will send a more detailed agenda when the date gets closer. Please email Megan Crowley at mcrowley@alamedahealthconsortium.org to express your interest in participating and to receive an Outlook invite by April 7. You may attend this event without having been to previous sessions. \nCHCN/AHC health centers are consistently faced with high numbers of patients with trauma\, depression\, anxiety\, and substance abuse disorders. This series will conclude with an overview of the current state of knowledge about types of trauma\, and the trauma-informed movement as it relates to health and health center professionals and staff. \nLearning Objectives:\n1. Review definitions of trauma and trauma informed care\n2. Review how childhood trauma results in adult disease and poor outcomes\n3. Describe ways of addressing trauma in health care\n4. DESCRIPTION:Please join us to kick-off our new PHASE initiative for 2017-2019. \nAt this meeting\, we will focus on the goals and strategies for improving cardiovascular health for our patients during the next three years. \nAs with our previous grant round\, Kaiser Permanente will require quarterly quality measure reporting at the clinic level\, and CHCN will offer incentives funds for site-level participation and meeting site-level quality goals. \nWe will base PHASE site level participation on attendance of clinical/operations dyads or other pertinent care team members at the kick-off and on the on-time quarterly submission of site level hypertension blood pressure control data. \nIn addition\, CHCN will highlight some final achievements from the 2015-2016 PHASE program. \nWho should attend: \n\nClinical and Operational PHASE leads\nQuality Improvement Leads\nCare team staff working on PHASE at the site level\nAnyone involed in improving cardiovascular health for their patients!\n\nSnacks will be provided.
