(Alameda County Coordinated Early Services System for HIV Primary Care)
HIV ACCESS is a primary care network serving people with HIV in Alameda County. Federally funded by the Ryan White Program Part C, its mission is to provide coordinated, comprehensive, quality HIV primary care services for Alameda County’s underserved communities. Tri-City Health Center is the RW Part C Grantee. The program encompasses six freestanding community-based primary care clinics, as well as two outpatient clinics operated by the Alameda County Medical Center.

Care sites are strategically placed throughout Alameda County, providing easy points of entry into the system. The participation of hospital-based clinics improves patient continuity and coordination during transitions between inpatient and ambulatory care. The Alameda Health Consortium provides coordination of program activities, including program planning and development, training, monitoring, technical assistance, and maintenance of the data collection and reporting system. Since the implementation of HIV ACCESS in Alameda County, accessibility to primary care services for people with HIV has improved dramatically.
