The Alameda Health Consortium comprises eight community health centers that operate nearly 35 clinic sites in Alameda County. Our eight member clinics each have a rich history of providing health care to Alameda County since the establishment of community-based health clinics/centers in the 1960s and 1970s. Despite their variety, there is one thing all of our clinics have in common; the provision of quality health care to those who need it. Our clinics serve diverse populations ranging from African Americans, Asian Pacific Islanders, Latinos, Native Americans, to seniors, immigrants, and the homeless.
Our member health centers are: Asian Health Services, Axis Community Health, La Clinica de La Raza, LifeLong Medical Care, Native American Health Center, Tiburcio Vasquez Health Center, Tri-City Health Center, and West Oakland Health Center.
Together these clinics serve over 100,000 people throughout Alameda County. The large majority of patients our clinics serve are low-income and uninsured. As health care “safety-net“ providers, Consortium health centers serve patients without regard for the patient's ability to pay. The Consortium health centers provides a wide a range of services including: primary care, pre-natal care, dental services, HIV support services, behavioral health services, health education, and social services support.

- All clinics provide primary care (pediatric services, prenatal, general adult medicine), health education, support services and other services in the community
- Services are available in a variety of languages
- All clinics have eligibility and enrollment services to link individuals with health insurance programs
- All clinics participate in Consortium-wide chronic disease management programs focusing on diabetes and asthma
- Clinics have specialized services in a variety of areas including hepatitis C, HIV, rheumatology, TB, teen health, acupuncture, substance abuse recovery, optical, and geriatrics
- Four clinics provide mental health services
- One clinic has a fitness center
- There are over twenty-two comprehensive medical clinic sites in Berkeley, Oakland, Hayward, Union City, Fremont, Pleasanton, and Livermore
- Six clinics currently provide dental services
- Our clinics provide more that 33,400 prenatal care visits per year
- Approximately 10% of all pregnant women in Alameda County receive prenatal care at our clinics
- We have eight school-based clinics in Alameda, Hayward, Oakland, San Lorenzo and Union City
- Medical services are provided not only at clinics but also at Single Residency Occupancy hotels, senior centers, and nursing homes
